Claiming 2.0 hours account on (DATE), I was assigned to (ASSIGNMENT) at (LOCATION). On (DATE) I was instructed by (UP MANAGER OR YARDMASTER) to ride the side of a car (CAR NUMBER) for (DISTANCE) with cabooses, shoving or riding platforms available.
