Insufficient Meal Period

• Location meal period occurred (if applicable)

REQUIRED CLAIM LANGUAGE:
Claiming 8.0 hours on (DATE) account not allowed a sufficient meal period. I was on duty at (LOCATION) on yard assignment (ASSIGNMENT) when I was instructed by (UP MANAGER or YARDMASTER) at (TIME INSTRUCTED) to observe my meal period on the locomotive. I was given an insufficient meal period which caused me to observe my meal period on the locomotive on the working lead while performing service.

Claiming 8.0 hours on (DATE) account not being allowed a 20-minute lunch period. I was on duty at (LOCATION) on yard assignment (ASSIGNMENT) when I was instructed by (UP MANAGER or YARDMASTER) at (TIME INSTRUCTED) to observe a meal period. I arrived at the crew room at (TIME ARRIVED) and I was instructed at (TIME) by (UP MANAGER or YARDMASTER) to resume work.
